Support Provider

This is an hourly position, full and part-time, reporting to the Director or designee.

Responsibilities:

The following is a comprehensive list of responsibilities for this position. The Specific Skill Set Assessment tool is used to make initial staff skill assessment and/or assess individual service needs. Staff will have responsibilities from the list depending on whom they are supporting.

    A. Relationships/Community Networking

  • Assist individual with developing supportive relationship when/where appropriate
  • Assist individual with locating and utilizing community resources and activities (i.e. social clubs and events)
  • Assist individual with locating sites for volunteer opportunities
  • Provide individual with various forms of companionship
  • Act as a community resource to individual/family

    B. Practical Supports/Skills Training

  • Personal care may involve but not be limited to activities of daily living. This support may also be in the form of teaching, prompting, and/or offering assistance in the following areas:
  • Personal hygiene (bathing, toileting, hair care, brushing teeth, etc.)
  • Laundry
  • Errands/transportation
  • Homemaker/housekeeping
  • Meal planning/preparation/nutritional information
  • Eating
  • Shopping
  • Child care
  • Home safety (recognizing, eliminating and correcting safety hazards)
  • Scheduling and attending medical appointments

    C. Communication

  • Maintain and submit accurate records (time sheets, progress notes, etc.)
  • Notify supervisor of any needed changes to assignment
  • Actively participate in team meetings, training sessions and conferences

    D. Physical Duties
    Provider must be physically able to accomplish the following, as necessary:

  • Assist individual with transferring to and from bed, chair, and wheelchair
  • Assist with walking
  • Range of motion activities
  • Positioning
  • Use of adaptive equipment
  • Lifting (weight depends on the specific client situation)

    E. Medical/health Issues
    Assignment may require knowledge or experience with the following:

  • Seizure disorders
  • Allergies
  • Medication administration
  • Assistance with deaf or hard of hearing individuals
  • Physical disabilities
  • Frail individuals
  • Sight impairments
  • Dental concerns
  • Respiratory difficulties
  • Heart problems
  • Blood-born diseases (i.e. Hepatitis B & C, HIV, Syphilis)
  • Communication disorders

    F. Mental, Social and Emotional Health

  • Assignment may require knowledge or experience with the following:
  • Alzheimer’s Disease/dementia
  • Chronic alcohol/drug abuse
  • Serious and persistent mental illness
  • Physical/sexual abuse
  • Behavior management (dealing with challenging behaviors, providing crisis management)

    G. Financial Support and Assistance

  • Set up banking services
  • Assist with money management
  • Locate representative payee
  • Secure subsidies (i.e. HUD, fuel assistance)

    H. Vocational Support

  • Assess individual’s job interests, skills, work history
  • Assist with development of a job search plan and develop outcome-based performance objectives
  • Assist with creation of resume
  • Assist with active job search
  • Facilitate appropriate job match
  • Provide on-going support to individual and employer

    Provide on-site job support including:

  • Role model work ethic and job skills
  • Foster relationship with supervisors and co-workers
  • Develop natural supports
  • Facilitate appropriate environmental modifications that support individual’s learning style
  • Develop “fading” strategy from worksite
